Tracking Code 358-260 Job Description Position Summary The Diabetic Education and Empowerment Program (DEEP) Coordinator provides leadersh! ip and oversight of the chronic disease care nursing model as they work in partnership and joint accountability with other staff members to achieve Neighborcare’s Mission, Guiding Principles and goals. The primary focus of the DEEP Coordinator is to optimize the health status of patients by providing intensive chronic disease management and patient education to our patients with diabetes by leading a team of 6 or more RN and lay health coach volunteers to do patient outreach and case management to the fulfillment of the two-year Attorney General’s Office Diabetes Coaching Project grant. While the DEEP Coordinator will be assigned to the Rainier Park and Rainier Beach clinic sites, they will also work in partnership with the Primary Care RN team and Certified Diabetic Educators to meet the needs of patients throughout Neighborcare. The DEEP Coordinator will primarily work to develop and sustain a diabetes coaching model that can be replicated at other medical clini! cs serving low-income diabetic patients.

The DEEP ! Coordinator follows the Nursing Process: to assess, plan, implement and evaluate nursing care. This position is currently grant-funded for two years. Continuation of the position depends on availability of future funding after the initial two years. Project Goals: 1.

Improve health outcomes for at least 600 diabetic patients through diabetes coaching. 2. Develop a volunteer diabetes coaching model that can be replicated at other medical clinics serving low-income diabetic patients. Project Outcomes: 1.

Neighborcare expects to see an improvement in diabetes control among participating patients with a Type 2 diabetes diagnosis at the Rainier Park and Rainier Beach clinic sites, as measured by HbA1c levels. The specific internal goal for Neighborcare’s clinics is 50% of the diabetic patients will achieve an HbA1c level of 7% or less, with a focus on achieving reductions beyond HbA1c (specifically targeting blood pressure and LDL cholesterol) to promo! te optimal health. 2. Neighborcare expects to recruit at least six volunteer diabetes coaches (RNs and lay health educators), at least one of whom will be trained in Motivational Interviewing and at least one of whom will be bilingual/bicultural.

Responsibilities Working in Partnership and Joint Accountability Provide leadership for and implementation of the chronic disease care nursing model for greatest benefit of patients Work with other clinic leaders to develop, implement, standardize or improve systems that will enhance chronic disease care management Ensure that Neighborcare has sustainable systems for orientation, training, mentoring and staff development for volunteer RNs and lay health educators Provide direct supervision of and training to volunteer RNs and lay health educators Manage defined panels of patients with diabetes at the Rainier Park and Rainier Beach clinics and work collaboratively with Neighborcare’s Primary Care Nursing Team Maintain ! joint accountability with the clinic team and with the medical program ! as a whole, to include working at a variety of sites to best meet the needs of the patients Reassess priorities as needed. Work with others to define priorities. Support colleagues throughout the organization either on site or remotely Work in partnership with care team members to plan and communicate care delivery Report directly to the Primary Care Lead RN with regular meetings to ensure the fulfillment of project goals and outcomes Chronic Disease Management Identify patient goals for improving their health status and management of diseases. Assess patient understanding of their disease and readiness to change Provide patient with education about the disease process and offer tools and skills needed to achieve self-management goals Coordinate care according to EHR (Electronic Health Records) chronic disease templates Establish partnerships with patients and families where appropriate for positive outcomes in health status Provide appropriate monitoring, tracking, and fol! low-up with patients Coordinate care with outside provider(s) and resources Utilize organization guidelines, protocols, and best clinical practices Patient Education Educate patients in self-management of chronic disease Educate patients in the use of diabetes medications Promote wellness and preventive care Provide motivational interviewing and harm reduction counseling Physical Requirements This position may require lifting and moving up to 25 pounds Requires prolonged standing and walking Involves over 2 hours/day average sedentary work at computer terminal.

Possesses manual dexterity necessary for keyboarding, data entry, writing, filing, and using phones This position carries moderate risk of exposure to infectious agents, including HIV, Hepatitis, and TB HIPAA Requirement HIPAA law and Neighborcare’s policy protects the confidentiality of all patient information. Immunization Requirement Staff members must meet immunization requirements for exposure to blood borne pathogens and TB as stated in Neighborcare’s immunization policy.
