SUMMARY

The RN Disease Manager is responsible for the evaluation, education, clinical intervention and chronic care management of those patients who are identified in need of management through our population based programs. The RN Disease Manager will assist in the management chronic illnesses for health plan enrollees by initiating a patient- centric plan of care that will maximize the patient’s ability to self manage their disease process using clinical assessment, evaluation, and motivation/intrinsic interviewing skills to facilitate the continuum of care utilizing advanced nursing knowledge, experience and skills to ensure patient quality outcomes. The RN Disease Manager will provide telephonic outreach, disease management, education and support to health plan enrollees in an effort to support healthy lifestyle choices and reduce long term effects of chronic illnesses. This individual performs other clinically based activities to coordinate care between enrollees, providers and community based health care providers or organizations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides telephonic outreach in disease management , education, enrollement and other clinically based activities to plan enrollees
  • Identify, conduct assessments, develops treatment plans and interventions and set goals for treatment plans/behavioral modification within the scope of practice for enrollees of the health plans.
  • Collaborates with enrolles medical/health and community based providers (including PCP) regarding the members treatment needs and plan of care.
  • Maintains clinical data and enrollee information for reporting and tracking metrics.
  • Monitors and evaluates the services and community based resources necessary to respond to the individual enrollees health needs
  • Completes other special assignments as assigned.
PREFERRED SKILLS · Knowledge of advanced clinical standards, practices, and procedures for designated patient population for specific chronic illnesses management such as (Diabetes, Heart Failure, Asthma).
  • Strong clinical assessement and critical thinking skills necessary to provide disease/case management services appropriate to clients with complex medical, emotional and social needs.
  • Ability to identify learning needs, set goals and seek educational opportunities.
  • Knowledge about Medicare, Medicaid, and third party payment regulations as they relate to patient health care needs.
  • Knowledge of individual and family development over the life span and the influence of cultural and spiritual values in health care.
  • Knowledge of available health care and community resources appropriate for community served.
  • Knowledge of, and skill in, the use of personal computer and soft ware applications such as the Microsoft Office Suite.
  • Knowledge of age specific communication needs with the ability to listen actively and respond to internal and external customers in a timely, competent manner both verbally and nonverbally.
  • Interface with multidisciplinary team to achieve outcomes consistent with the members plan of care.
  • Ability to see the global picture and through the disease management principles of assessment, planning, intervention and evaluation provide proactive and concurrent management of identified populations in improving health outcomes.
  • Ability to make on site visits, in the hospital, skilled nursing facility or home setting for the purpose of coordination of care and fulfill individual plans of care if necessary.
  • Ability to provide own transportation.

  • Ability to use tact and diplomacy in interactions with internal and external contacts.
  • Ability to work cohesively with patients, physicians and medical staff to articulate complex concepts in managing chronic care principles.
  • Skill in writing clearly, grammatically correct, user friendly instructional documentation for the staff.
  • Ability to analyze problems and formulate plans, solutions and course of actions.
  • Communication skills to convey terminology to staff of varying levels of expertise.
  • Demonstrated skill in applying accounting and auditing principles and practices.
  • Ability to work with frequent interruptions, manage time, work under pressure, manage deadlines, set/change priorities and use good judgment in daily work schedule.
  • Ability to maintain confidentiality of patient and NPN business matters.
  • Ability to work accurately with detail.
  • Knowledge of technical resources within the organization for needed information
  • Ability to judge appropriate information to be conveyed to different levels of management.
  • Skill in working as a team; collaborating with colleagues.
  • Ability to establish and maintain cooperative working relationships with individuals at all levels of the organization.
KNOWLEDGE, SKILLS, AND ABILITIES

Language Skills: Ability to read, analyze, and interpret documents, reports, technical procedures, governmental regulations, and correspondence; ability to write routine reports and correspondence; and ability to present information in one-on-one, small group, and large group situations to management and staff. Ability to apply proper grammar, spelling, punctuation, and format to typed material. Must have excellent oral and written communication skills.

Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical inference; ability to compute discount, interest, profit, loss, ratios and percents; and ability to draw and interpret graphs.

Reasoning Ability : Ability to solve a wide range of practical problems and deal with a variety of concrete and complex variables; ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form; and ability to deal with multiple variables.

Demonstrated a bility to operate related computer applications (i.e. Microsoft Office Suite ), and business equipment including adding machine, personal computer, copy machine, fax machine, and telephone. Ability to operate a motor vehicle.

Knowledge of Medical Management policies and procedures. Knowledge of Soundpath Health’s products and services and related regulations that can be used to provide exemplary service.

Strong leadership skills. Excellent public relations and communication skills. Excellent conceptual, organizational, and problem solving skills. Well organized with the ability to maintain attention to detail. Ability to work well in stressful situations.

Education and experience: Bachelor Degree of Nursing Required, MSN preferred. Two years Disease Management or Case Management experience .

Certificates, licenses, registrations : Current unrestricted RN License for Washington State. Case Management Certification preferred.

PHYSICAL DEMANDS Talking: Especially where one must frequently convey detailed or important instructions or ideas accurately, loudly, or quickly.

Average Hearing: Able to hear average or normal conversations and receive ordinary information.

Finger Dexterity: Using primarily just the fingers to make small movements such as typing, picking up small objects, or pinching fingers together.

Repetitive Motion: Movements frequently and regularly required using the wrists, hands, and/or fingers.

Average Visual Abilities: Average, ordinary visual acuity necessary to prepare or inspect documents or products, or operate machinery.

Physical Strength: Frequently required to sit, stand, and walk and occasionally required to stoop, kneel, crouch, or crawl. Occasional ly required to lift and/or move up to 20 lbs.

YOUR WORK PLACE Internally, work is normally performed in a climate controlled office environment, where exposure to conditions of extreme heat/cold, poor ventilation, and fumes are very limited. Noise level is moderate and may include sounds of normal office equipment (computers, telephones, etc.). No known environmental hazards are encountered in the normal performance of job duties.
